Unfortunately, it seems P-Hoff had fallen off the wagon in the weeks leading up to his untimely passing, and according to friends, he even predicted his own demise.

Philip Seymour Hoffman was reportedly was on a six-week heroin binge before he was found dead in his New York City apartment this past Sunday.

TMZ reports:

Sources Hoffman confided in tell us … back in December Hoffman told them he had started injecting himself with heroin and couldn’t kick it.  Hoffman said he would kick it for a few days and then fall off the wagon.

During the last 6 weeks he often look disheveled and dirty. At one point someone asked him how bad his problem was, and he responded, “If I don’t stop I know I’m gonna die.”

Sources also claim that Hoffman was excessively drinking, which led to many bad decisions when it came to his heroin use.

The Hollywood Reporter reveals that the celebrated actor only had one scene left to film for The Hunger Games: Mockingjay- Part 2, slated for release on November 20, 2015.

Lionsgate insiders say the late Philip Seymour Hoffman had one major, emotional scene left to film in the final installment of the Hunger Games series, The Hunger Games: Mockingjay – Part 2, but they express confidence that filmmakers can complete the movie in a way that will satisfy fans.
